π³ Outpatient Treatment (Doctor Visits, Tests, Procedures)
For outpatient care, you must pay upfront and submit a claim for reimbursement. Your doctor's invoice must include:
β Your name
β Diagnosis
β Detailed breakdown of medical services
β Treatment costs
β οΈ Pre-Approval for Outpatient Treatments
Some outpatient treatments require prior approval, especially high-cost or complex treatments.

β For outpatient costs of β¬1,000 or more, you must submit a cost estimate to Squarelife at least 5 days before treatment.
β Failure to get pre-approval may result in only 50% reimbursement.
