🌍 No Network Restrictions – Choose Any Provider

You are free to visit any doctor, clinic, or hospital worldwideβ€”whether public or private. There is no restricted provider network. However, for certain outpatient treatments, you must obtain prior approval from the insurer to ensure coverage.

To receive full reimbursement, you must get prior approval from Squarelife before undergoing:

βœ” Elective surgeries (planned, non-urgent procedures)
βœ” High-cost outpatient procedures (€1,000 or more)
βœ” Specialist consultations for new, complex conditions or second opinions

βœ‰ How to Request Prior Approval

To request approval, email [email protected] with:

πŸ“Œ A brief description of your medical condition
πŸ“Œ Details of the proposed treatment/procedure
πŸ“Œ Medical documentation from your doctor/hospital

⚠️ Important: If you do not obtain prior approval, only 50% of the covered costs may be reimbursed.

After choosing your provider and receiving treatment:
