Planned surgeries that are not urgent.
High-Cost Outpatient Procedures
Treatments or procedures with estimated costs of β¬1,000 or more.
Specialist Consultations
When the nature of the condition is new or complex, or when seeking a second opinion.
Also required if the cost of the treatment is high.

Relevant medical documentation from your doctor or hospital
